Our commitment

First Horizon Behavioral Health and Wellness is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of your protected health information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

  • As required or permitted by law — including public health activities and safety concerns.

  • To request access to and a copy of your health records.
  • To request corrections to your health information.
  • To request restrictions on certain uses and disclosures.
  • To request confidential communications.
  • To receive an accounting of certain disclosures.
  • To receive a paper copy of this notice upon request.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future.
